I would suggest that you first consult with your surgeon as to your travel plans, and what is contraindicated regarding your back history as a patient.
L-3, l-4, or l-5 history, fusions, or laminectomies all require a different approach to pulling, lifting, and carrying weight. The added eventuality of bending over to attach, or remove a rolling cart for your luggage also presents risk to your back, post surgery.
Adding possible rotator cuff problems with the shoulder increases the dimension of re-injury or damage to your shoulder, and (if carrying or lifting luggage) possibly your back.
I believe your surgeon would agree that any travel case that is easy to push or pull without struggling to much would be a good choice. For that reason I would suggest any small, lightweight, well-built case with a good set of 4 wheels, and having a taller pull handle to avoid bending over while pulling is better.
I would suggest a Rimowa carry-on four wheel model, such as the Salsa series, which you push ahead of you with two fingers rather than pulling, the merits of which you can discuss with your physician.
