Origin Pacific in New Zealand. Not actually doomed from the start, but doomed the instant it signed a deal with the devil.
Started out as a freight operation and then added some pax flights. All going well and reasonably pleasant to fly. Then it signed a code share deal with Qantas. Part of the deal was that it would buy, purely on it's own dime, half a dozen new large aircraft, but it forgot to get a guarantee that the relationship would continue. QF then promptly pulled the plug on the codeshares and Origin went into receivership within ~72 hours as they suddenly had all these planes and no slots to fly them on. (Technically they stuck around for ~18 months, but they were a zombie airline almost immediately after QF stuck a knife in their back.)
