Certainly the top two are perfectly ethical, the bottom one depends on the individual's ethical basis, but at least the airline gets the value of the money between the purchase and cancellation date.
Conveniently you didn't mention buying from John Lewis using Paypal and getting a refund via the card, without ever having the intention of keeping the purchased goods.
As for buying refundable tickets, if you never had the intention of flying in the first place, I think that fits pretty much the definition of fraud.
