Sorry, but the honor of biggest embarrassment has to go to Air Canada and its ridiculous Aeroplan program (widely known as Errorplan ... enough said). No other airline or FF program has treated its best customers with the absolute and utter contempt that these folks have.

Having said that (and sorry for the mini-flame) Delta has been an absolute treat to deal with on every level. Moving my business to Delta from AC was the best move I ever made. Thank you DL!
