Let me close the loop about the $28 puzzle; to include or not include a prepaid return envelope. Couple of weeks ago I had posted same questions like everyone else.

So:

(1) Like others I filled in the BLS orde form, and received and emails with following information, but no FedEx label

CARDHOLDER NAME :#######
CARD NUMBER : ################
DATE/TIME : ##########
REFERENCE # : ### ####### #
AUTHOR. # : ######
TRANS. REF. :

(2) I went ahead and submitted the passport application for visa with (a) note that $28 has been charged, and I assume that BLS will take care of return shipping, and (b) included a printout of the email

(3) Couple of day ago I got the passport back (with visa) via FedEx, and everything was done

Once the passport got to BLS, I was pleasantly surprised by their efficiency and online tracking. Only of the BLS side had explained the purpose of $28 and what it was for.
