If I am staying in a mixed hotel, I generally wouldn't call down about a nighbour smoking on their balcony - after all, it may be a smoking room. But if I were in a 100% smoke free hotel, where it is against their terms, then yes, if it were spoiling my enjoyment I would let hotel staff know.
And it if was not bothering you in the lightest bit, would you STILL call down and report it, like a hall monitor in high school?
I just received this reply from one of my favorite hotels in Key West, who has recently gone non-smoking:
We are a non smoking hotel but do have 10 balcony rooms where you can smoke on balcony only. Veronica, pls advise them of the balcony room rate over the dates they request.
