I think this is spot-on. OP may be conflating the different types of points since OP references only "points" and "companion pass and A list preferred" without differentiating between Redeemable Points, CPQPs and TQPs. I'm guessing the "58k points from flying" OP mentions includes some of the promotions that offered 2x of different types of points (i.e., redeemable and not 2x CPQPs) and OP didn't make the 50k TQPs or 110k CPQPs needed for A-List Preferred and Companion Pass qualifications, respectively.
