I think this is a great point, and one that is rarely brought up. If you're on a plane for a long time, chances are you'll need to use the lavatory. Foreign carriers always seem better on this point, and just on cleanliness in general.
The only thing that would sway me towards AA is the inflight wifi, I'm just not a big movie watcher.
