The most common mistake is entering China as the destination country instead of the transit country. As for AA, we seem to continue to get sporadic reports about their agents' inability to find this on their database--which means either their database is incorrect or their agents are clueless. TWOV isn't exactly new, it's been around a few years even in abbreviated form.
