We've taken our daughter (now 4) on 47 flights to 7 countries on 3 continents, including Canada, Spain, France, UK, and Israel. We have not had any issues.

Only once - before the adoption was final - the US Customs agents (upon our return to the U.S.) asked (appropriately) to see the court order allowing out-of-state travel.
