When I was last there, I was excused the fee for one extra guest, which was perhaps because I was diamond. As for other benefits, free breakfast is for two people and the hotel provides free internet to all guests anyway.
4. This question has been asked and answered many, many times in this thread. "Best" is in any case subjective and depends a lot on your relative values, time of arrival, amount of luggage carried, and feelings about using public transport. A common recommendation is to use the Narita Express to Tokyo station and pick up a taxi. A taxi costs around ¥2,500 depending on traffic but if there's five of you you'll need two. The Narita Express has very recently launched a special fare of ¥1,500 per person from the airport into town for foreigners.

Cheap and easy. From Ginza I took a taxi for about 1500Y to the Conrad. "Hoteru Conrad-o Shiodome". A google maps image of the hotel might help, too.
