Another data point: the coach flight was with a Trek 5700 triple that I bought from a nice little shop in Redwood City. The 1st class flights were with a Cervelo P3C. You can develop your own theories around this.
I have actually had an agent forget to charge me once while traveling with my bike. And that's out of probably 20+ times.

You've just been very lucky as fare class has no bearing on waivers for shipping a bike box.
