You can also go to http://www.united.com/refund and enter the details. Do NOT expect a quick response from this, but once you've filled out the details the refund will happen.

I've had similar refunds take anywhere from a few hours to a few weeks. Given the number of refunds they are likely processing after last weeks storms I wouldn't be surprised if it's near the higher end of that at the moment.
