I've flown two round trips with my bike in domestic first and two in Global first and wasn't charged. In the case of the domestic flights, the saving on bike fees roughly equaled the first-class fare premia. I did get charged the one time I flew in domestic coach.
Another data point: the coach flight was with a Trek 5700 triple that I bought from a nice little shop in Redwood City. The 1st class flights were with a Cervelo P3C. You can develop your own theories around this.
