Do a search using the keyword(s) "conditions of carriage" or "CoC" in FT parlance, limiting the forums to AA, UA, DL, AC, etc. I got in a hot row with some of the guys on the AC board a couple of years ago when I was arguing this point (from the 'no-one-gives-a-fart' perspective). I had plenty of examples thrown at me. I think there is at least one UA (or AA?) example of having his account closed with all miles lost.
