They're not dead, they have just reduced the number of phones per airplane.
A narrowbody will typically only have one in some central location. Delta's DC9 varients (DC9-50, MD88, MD90) have one in one of the forward closets. United's 777s have two in the aft galley and probably others farther forward.
The ones on the United 777 showed a rate of $10.00 to setup the call plus $10.00 per minute. Seemed a bit steep to me.
