I'd start with calling AA. As this is an AAdvantage issue:

U.S. or Canada
AAdvantage Customer Service
1-800-882-8880 (or your Elite Service Desk telephone number)
9:00 a.m. - 5:30 p.m. (CT) Monday - Friday
The company that AA has publish and distribute their cards etc. seems to have messed up on this one; many others merely have not received any card, etc. upon qualifying in the last couple of months.
