Each of the last 2 years I have worried about requalifying but have made it. I have read about people who have flown 200,000 miles but have not made it do to a low spend. $50k seems to be the cut-off, even though there is no really a known bright line for this. The only way to prove this (wrong) would be to find someone who spent more than that and did not make it.
This is me last year. I did ~260,000 on UA metal but my spend was ~$35,000. (I didn't keep track of spend last year).

Most of my tickets last year were international (China mostly) M, S, H fares. I think I had one Y and one B for 2012.
This year my flights have been 100% Y, B or M and I'm close to $35,000 while only doing ~102,000 BIS (168,000 PQM).
