1. What advertisers' banners are on the page at the time the survey appears?

2. What is the survey asking about when you click it?

3. Of course, a screen shot of the survey and/or the whole page would be useful.

In order to block unwanted ads, the more info we have about what is appearing, the better we can trim the bad surveys. Thank you for the report.
