Lets face it, there's more than ample time to request offloading before the passenger makes it to their seat onboard.

If they've waited till the last minute, then its their problem, not the crew's. Medical emergency is one thing... but a simple desire to be offloaded is not particularly reasonable when you consider everything involved with that from the airline's point of view.
If the flight is stuck on the ground with a slot time in 2+ hours surely at some point refusing to allow offloading would become detaining someone against their will?
