I think your itinerary is pretty solid. Are you planning on having a guide in Chiang Mai? Having access to transportation will help open up Doi Suthep and Doi Inthanon (highest point in Thailand) along with elephant and raft rides. Make sure to find some Khao Soi, a Northern dish of burmese origin, while in Chiang Mai.
