The form I got was a tri-folded sheet that was square shaped. The inside pages allowed me to sign up for the other programs (SPG, Accor, etc) and the back page was CX. I didn't actually use the other pages since I did the sign ups over the phone, but for CX I had to send the form in for processing. It was a relatively quick process as I received the CX gold & luggage tags before I received Accor & Carlson packages even though those were done over the phone.
