Change the METHOD to [Awards-Upgrades/UA] on the 'Awards/Upgrades tab'. I think it defaults to [Awards/StarAlliance] -- or that is what I had selected.

Finally I found the "method" window and things are working well. The only remaining problem is ....

KVS sent me my diamond credentials overnight, but I did not get the registration number in the email. That's pretty frustrating, but easy to fix.
