FYI - you are unlikely to get a refund from CVS and will waste a lot of effort trying. The refund needs to come from Incomm. CVS sold and activated a valid card (your receipt and their records prove it). They did their bit. The fraud control side of Incomm is what let you down.

Other reports like this over the last year or so have eventually been resolved by Incomm issuing a new pin or simply applying the credit to its intended destination (BB etc.)

I'm going to go find out when the manager is on duty...and have a talk. Hopefully, this is something he did. Because I'm really hoping that this wasn't a security breach.
I'll get him to call InComm and verify that each card is inactive. And then make him just refund my card. Because at this point, I'd rather just have the money back safely.
