It's always bad when this happens, and I have seen it regularly on SIN (almost every flight?) and sometimes in HKG and often in LHR T5 too (though the circumstances are different there). The annoyance and upset it causes is considerable, especially if there was something like an irrop that meant that a passenger ended up connecting rather going direct.
In T5 WorldDutyFree they do check boarding passes for this, and if it looks like there's an Australia trip they will advise you. A message also pops up on the screen. I heard this happen to someone a few days ago and that passenger was quite surprised to learn about it. The airside luggage check mechanism is from the two sets of customer service desks by gates A10 and A14.
It's not foolproof, not all shops have this check and also if you present a SIN boarding pass it is not obvious that you may have an onwards connection.
