1. No; you can get a refund less 10% ($5 minimum) or an eVoucher for 100% good for a year.
2. Yes; you will not receive AGR points unless your ticket is scanned.
3. Yes; with unreserved trains, it doesn't matter whether you do this before or after the train departs. You can get a refund or eVoucher (or just use the unreserved ticket on another date--you don't even need to change it) at any time up until one year out.
Personally, I think it's dumb that Amtrak charges a refund fee for unreserved trains (what are they losing, other than a few cents of a credit card swipe fee?), but with the move to the ability to apply eVouchers online, I'm far less
