Following up on my own post in case anyone is in a similar situation. Mrs. FindAWay applied on-line using the targeted 100k offer link and code. There was a "Name on Card" field in the on-line application that we completed with "Mrs. FindAWay" and the card came with that name on it.

HOWEVER, her emails from Amex, her on-line account (it was her first Amex), and the RECEIPTS printed out by merchants have "Ms. MaidenName" on them. It is quite odd that the receipts merchants print out have a different last name than that printed on the card; but, no problems so far. Once the bonus hits, we'll send in the marriage certificate to get the account changed over to her married last name.
