No, it's not a "money grab." It's a way to price discriminate between leisure travelers and business travelers. I.e., extract top dollar from each customer at time of sale. It's not about collecting the $200 for the trouble of making the change, but rather to force people who are likely to change travel plans to buy more expensive refundable tickets to begin with, as they have deeper pockets to do so.
It's the equivalent concept to the mattress store who gives you a credit for your old (worthless) mattress; coupon clippers; etc.
