Oh, and DEN-COS (73 miles). Did UA ever fly MKE-ORD (67 miles)?
UA flew mainline on ORD-MKE; they also flew SFO-SJC with a 727. I was also once on a DC-8 in the 1980s from ORD to Sacramento that continued onto SFO.

Not UA, but TWA used to tag on several flights from ORD to Rockford in a 727 back in the days before they had buses.
