Like I said, everybody pays the fee. But it is good to know that there is a way to avoid the immigration/emmigration lines. Edit: the main fee payment booths are outside im/emmigration. The transfer desk is inside.
The fee has been payable using credit cards since last year. (I didn't have to pay it though when I was transiting international-domestic in CEB; I wonder if such is also the case at MNL.)
Also, if I recall, those guards check the boarding pass to make sure you're at the right wing, not to check whether or not you've paid the fee (as everyone in theory who's in the pre-departure area has already paid the fee).
