If I do miss the last tube, what would be my best option for getting from the airport to Southwark (around Guy's hospital)? Are there any options other than a taxi? And would this be something BA might be willing to pick the bill for, considering the late departure and arrival? I'm hesitant to use the night buses, as I'm a single woman traveller with some past unfortunate history of unwelcomed attention on one of those.
Minicabs would be much cheaper and frankly more comfortable than a black cab (I find their suspension to be poor, and the way seats on black cabs are, you will feel the need to grip the floor more, which you may not like with a bad ankle).
If you search for Heathrow Airport Minicabs on Google you should be able to bring up many and compare rates pretty easily.
