I had a puncture out of DEN a few years ago and the Chevy Cruze has no spare, just an 'air pump.' So, I put more air in every few exits until I arrived at the hertz location and they swapped the car for me. The hertz guy asked why I didn't call for assistance, I explained that I was NOT willing to wait in the middle of nowhere for hours to be "assisted." I was not charged for a puncture as it was no fault on my part and I did not have the LDW or any other upcharge from hertz.

I would expect if you damage a tire through recklessness or neglect then you must pay, but for a puncture on the highway, no way would I ever pay.
