Your policy will normally define what happens in terms of departure (from residence, initial airport, first international, etc) or arrival at the destination (similar variation) you have told them about. If you're covered after departure then you will probably be OK. Of not until arrival then probably not.

The bigger problem you may have is that if a pre-existing condition makes it likely that you will become ill on the journey then it may not be covered at all.

As others have said, read the policy carefully and assume the insurer will take a narrower rather then broader interpretation. If it is in the UK then the Insurance Ombudsman may interpret how to apply in unanticipated scenarios more in your favour, assuming you have acted in good faith.
