You used to be able to add frequent travel programs after the rental at avis.com. I did it several times.
Sometimes it is well worth the fee when there is a promotion going on, like the recent AA and UA bonuses on Avis rentals. I had some multi week rentals, so those bonuses added up rather nicely.
