I used to work for a regulator in another sector but regulators based on inspection follow the same basic process.
Immediately after an inspection the local manager would be told of any any failings as well as the expectations for improvement (if necessary) as well as an indication of the rating to be given.
The written report always came later - and the premises owner was shown a draft during 'fact checking' stage. So BS really can not say they were unaware of the failings until they received the final written report.
