I think the word "foreseeable" would matter quite a bit in determining what is covered. If, for example, you went to Syria and were injured in the fighting there, you would not be covered because entering a country in the midst of a civil war makes being injured a "foreseeable" probability.
However the mall attack in Kenya was not "foreseeable" by the general public or from what I've read, by any government authority issuing warnings.
