Terrorist attack while traveling: Who pays for the hospital?

Please, let's not make this into a debate about ObamaCare -- it has nothing to do with it.

I have an excellent travel insurance policy issued by my German credit card. It pays for all health expenses I incur (outside of Israel, where I live, and Germany itself).

The exceptions are acts of war (which include terrorist attacks). If I am in Israel and injured by one, my Israeli insurance covers it, but not if I am out of the country.

In most cases, does travel insurance cover acts of war? If not, do the host nations pay for medical care/hospitalization? Or, in such instances, does the victim have to pay for everything himself?
