US Global Entry and the Dutch Privium are indeed pay for play for most. In the case of US CBP, it is the less frequent international flyers who seem to be taking up the enrollment slots more than ever before, probably because after waiting 20+ minutes once or twice a year and finding out that it comes with fast track security screening at most major US airports when flying on US carriers, they pay for it or get someone else to pay for it one way or another.
The UK plans on making people pay for it starting next year some time.
