There are cards that give you a limited number of annual Priority Pass passes (ie. Basic PP membership plus 4 passes annually). BMO World Elite MC at $150/year (free supp card) gives bonus signup BMO reward points, includes cancellation insurance, out of country medical coverage, CDW, etc. The new Amex Reserve Air Miles card also offers 4 annual PP passes for $299/year plus other benefits. Both offer additional lounge visits for US$27 per visit after first four are used. These don't work so well for frequent fliers who want regular lounge access.

Scotia Platinum Amex also has PP membership and access for $399/year.

None offer free first year however.
