I debated and debated and finally decided to go with an insurance inclusive (zero deductible) rate with Italy. I have both the Sapphire and Ink cards, but in the end it seemed like more hassle to depend on them for coverage (at least for Italy). I ended up finding a diesel A class via Kenmel for not much more than the rates I was getting with Hertz.

We've also had good experiences with Kemwel (and their parent AutoEurope). Both usually have rates in Italy inclusive of all insurance. But you still need to shop around - for a trip in November they were over $1000, and Hertz was $400 (without the extra insurance).
