I doubt the FA's have to, or even can enforce the policy - it's almost certainly blocked by the IP address, as I would assume are streaming video sites like netflix. It's probably both to reduce the disturbance for other passengers as well as to conserve fairly limited bandwidth.

Re bandwidth -- VoIP is < 64 kbps, it's a trickle. Originally it was all about not creating competition for a future (paid) roaming cellphone service (which will never materialize), but it became about pax not being disturbed. If you go far back enough, you will see threads about loud and obnoxious DYKWIA-types talking loudly on Airphones.
