I saw exactly the same thing this summer. The answer is almost certainly failure to spend money on the necessary software. The problem could equivalently be failure to staff the software department adequately. We see that with Southwest Airlines so much that we consider it part of their corporate culture to deliver inferior software long after it's needed.
