I called today (1-866-606-2961), and talked to one CSR about my application. He seemed to have a hopeful tone in his voice, but then we got disconnected. I called back and spoke to another CSR, who told me in no uncertain terms that a denial letter had been sent, and that of course I was denied, you can't have two of the same card, and you certainly can't get the same signup bonus twice. She had no explanation as to why I had been told I was approved, and no clue that Citi does (did) in fact give out multiple sign up bonuses.

I ended that call fairly quickly, and immediately called back to try another CSR (trying to get through before she had a chance to update her call log with what we talked about). I got another CSR, who was able to see my application, and see that it had indeed been approved. He said he had no way of knowing where the card might be (thanks USPS), but transferred me to customer care, so they could send a new one. They actually gave me a bigger number than anyone else had - they said it could take 7-10 business days for the card to get here, and didn't seem willing to send another one right now. I'm only at 11 business days, so I'm just to be patient. At least I got confirmation that there is an account there for a new card to be sent on.

tl;dr: Apparently the Citi CSRs can have multiple, sometimes conflicting, pieces of information in front of them. Their response may depend on which one comes up first, what kind of mood they're in, etc. If you're not getting anywhere at all, it may be worth hanging up and calling right back to try to talk to another one. This is, of course, only on things they may have some control over. The new limitations probably don't fall into that category.
Calling to confirm the bonus has never accomplished anything except confirming what you already know. Anyone with a very recent approval should, IMO, stay off the phone, spend the 3,000 within 2 months and watch the miles appear. If they don't post (They will.), THEN call.
