This is incorrect, it's by membership year. We used the companion ticket from my wife's card late last year (Thanksgiving IIRC) and then booked another in August of this year (our card year starts again in March).
My experience as well, having purchased a ticket on my Premier in May 2012 and renewal in December bringing with it a new companion pass. In fact since using the prestige pass to book this month, when I log into the site it says:
Your annual complimentary companion ticket has already been used for this membership year. It will be available again on your next Citi ThankYouŽ Card enrollment anniversary date on 06/30/14 . You can use your Discounted Air-fare an unlimited number of times per year
