I think that form has not been revised, last option is not valid any more. There has been recent changes in the Visa Merchant Account setup, now the merchant can be selective about cash/credit. If U file a complaint with OCC saying the Office Depot 'cash' policy is encouraging the 'money laundering' illegal activity, OCC can write them a warning letter! If Office Depot thinks buying debit/reload cards is 'abuse' or 'not intended' purpose of the credit card, they have an option to take it off the shelves! that's fair!
