If you did this UuA / MFU after you'd already been op-upped on LHR-SFO, I can imagine mechanisms that could explain it, like your suspicion, but I also suspect that it's likely to have been an unintended consequence. Whether one would want to ask for £60 back after having had an op-up on the outbound flight would, I think, be a matter of personal choice.
So I called the Gold line. The web site got it wrong and somehow was including my outbound flight which was a "discretionary upgrade" (U class). The person I spoke to initially tried to tell me that they couldn't help because it was U class and therefore they had no way of knowing what I paid. I persisted with the fact that I was W class both ways and the upgrade was by BA, not my TA. This they finally accepted and they have refunded me.
Given my longhaul flights are nearly always fully flexible W tickets (£££££), I don't feel bad about asking for the refund!
