1. If you pre-book the spa treatment you should be able to manage it.
2. JFK will give you the boarding passes for all of the return leg. You can still check in online and pick up your boarding pass.
3. Taxi fares with yellow cabs in NYC are fixed rate from JFK to Manhattan. Although please note the actual fixed fare that's displayed doesn't include the toll, this automatically pops on your meter when you pass the toll point. All yellow cabs take credit cards.
