Could it be that during check-in process the agent mistakenly thought that he/she has already placed the tag to DFW on the baggage which is missing, because there was already a tag on the baggage? Can be the lesson to remove all old tags before checking in for the flights. Agents are human, too, and that kind of mistakes can happen.
