Are you talking about body scanners or x-ray machines?

Body scanner images are viewed in a separate area - staff get a yes/no for further search.

X-Ray has automatic algorithms to flag objects, but staff can also manually flag too.
Staff 'should' go through lots of training and this will normally train the brain to easily spot objects
