I think in the premium cabin, that's where it should end- at the "thank yous".
Discriminating at meal times can be embarrassing to fellow travelers. For example, I was having a wonderful conversation with a gent I had just met on my way to AMS from EWR, when the FA comes to take our meal orders. She asked me, and I diverted the question to him, letting him give his choice first, just as a gesture of social etiquette. But the kind lady insisted, very politely to be fair, that she wanted my order first. I quickly figured that I somehow scored over him in the BE social hierarchy (paid BE vs Upgrade?) and not wanting to create any embarrassment for either party, went ahead and gave my choice. The FA then actually moved on, and came back later, addressed the gent nicely by name, told him what was left and took his order.
Needless to say, it left us both somewhat uncomfortable.
